no many are facing this issue, I would recommend you to use something like Google cam mod or use manual mode in stock camera, there just tap where you need to focus and click the pic.

You should contact customer support. There is a good chance it is a hardware problem and they will give you instructions how to send the phone for a fix.
In my case the whole service process took only five days, so thumbs up to oneplus for really nice response!

Kudos to OnePlus for repairing the camera on my 3T, out of warranty, absolutely free of charge. Apparently, the camera module in many 3Ts is defective. OnePlus is doing the right thing and replacing the defective rear camera under an extended warranty program. And what is even more remarkable is how quick the phone was repaired and returned. After getting the RMA from OnePlus support I was given a FedEx label for overnight shipping. I dropped off the 3T at the local FedEx office at 2:15pm on Tuesday. It got to the Acer service center in Texas on Wednesday morning, the next day!!! It was repaired and shipped out, overnight, the same day!!! It was delivered to my front door at 9:30am on Thursday morning, fewer than 48 hours after I brought it to FedEx...just amazing. Thanks OnePlus, it's great to have my phone back, didn't take me too long to get really annoyed with the old HTC M9 I used when my 3T was in the shop.

The issue popped up after recent OTA update. For me it'll focus once rotated. ie, If camera is launched in portrait try rotating to landscape and vice versa, then tap to focus. Worked for me every single time.
Hope it'll work for someone else too.

I have the same issue. i have factory reset it and this has not resolved it. been a loyal oneplus customer since OP1, this bug basically makes the phone unusable. i have tried the manual camera thing - that doesn't work. the factor reset didnt work. even tried third party cameras, they don't work either. need help!

After all those things failed to fix my 3T auto-focus, I had to raise a service request and they replaced the rear camera module. It's working like normal again now and everything about the software is the same as when it stopped working properly.

    Hi,
I had the same, well known, problem. I contacted support (just click on "support" on Oneplus's main website, choose "repair service" and write them about your issue), sent in my phone on Wednesday with UPS and it's already on its way back, repaired, everything free of charge! Kudos to the 1+ team!
    Good luck!
