I am seeing this call icon on volume control which wasn't there before. I am not able to hear ringtones or notifications due to this.
Restarting phone fixes the problem just for a while.

How do i fix it ?

try restting all app preferences, then restarting.
    settings>system>reset options>reset app preferences>reset apps
do tell if it returns again

Disabled Bluetooth, no external device connected.

What is the OOS version you have? Have you done any changes to developer settings?

I am asking because your volume control panel is different than mine

There it is. All up to date and never messed with developer settings. When i click the volume button it shows me this with call sign. It's always just the media sign here but due to this issue it's different. Still couldn't find way around

No, I was just trying to understand what you thought was difference except the 4th one.
I can get that as well when I call someone/or in a meeting room.

This is weird... Can it be because you are using feature like 'okay google' or something? Some Voice controlled app/feature which keeps running in the background?

Mine looked exactly similar before thks problem. It's like my device thinks I'm always in a call mode and that's the root cause. Tried everything i could but still unable to solve it. Driving me nuts..

I haven't installed anything new. I'm thinking the same that something like ok google or similar is what my phone thinks it's connected to. Not able to sort it out. Tried restarting multiple times, clearing cache, resetting app preferences, absolutely no difference whatsoever
