This might be a hardware issue with your camera. Factory reset your phone and if after factory resetting your phone the problem still persists, then you know it a hardware problem. Report that to OnePlus Customer Support and they will replace the camera for free.

Some OnePLus 3/3T models have been affected by this hardware issue. Yes I am sure as many people replaced it for free because they had a defective camera sensor.

It's a hardware issue. I had that with my OnePlus 3. Take phone to OnePlus service center for repair.

Looks like the focus part of your camera module is stuck somehow. Try waving a magnet around the camera lens of your phone to try to loosen the stuck focus part of the camera module. This is just a temporary solution though, until you can get it repaired in a service center.

@SoniaB may you escalate further? User has defective camera and OnePlus wants him to pay for the repair and parts.

had the same issue like 8 months ago. had to send in my device for repairs, seems it was a faulty camera module. i had my phone back in 3 days time. if your phone still has warranty left it should not cost you anything
