I had this problem, took a few tries to figure out workaround. It boils down to the order that you do things.

Doing things in this order allowed me to finally boot into TWRP recovery:
    • Picking up where you left off above, right after recover img was successfully transferred, type 'exit,' and the cmd window should close.
    • Unplug USB cable from OnePlus One. Phone screen should still say Fastboot Mode.
    • Hold power button, till device powers down.
    • Press and hold Power & Volume Down.
    Hope this helps someone!

COS12 will Always overwrite the recovery.
To avoid this you mus reboot directly in recovery just after flashing it.
Doing this the TWRP will automatically patch the ROM to don't revert to the default recovery.
To do this follow the above post by @martron

BTW this questio have been aske many times. try searching next time:)

I was having this problem in my xperia active(st17i) while installing cm12.1 over cm11.
I solved like this: I unpacked the both boot.img in android image kitchen
cm11 recovery was cwm6.0.5.1.( I liked this to retain over cm12.1)

In the unpacked files I replaced all files containing word "recovery" to cm11 to cm12.1 boot unpacking.

    Then repacked the cm12.1 boot.img.
Then I flashed the boot img via adb.
Then other flashings as usually required. cm12.1 zip and gap zip.

I found this by doing many things for 3 full days.
