So I had OPO since June 2015 and used it everyday to this day.

I'm sure the battery degraded over time but I want to find out the info like what's the current capacity of the battery working left etc if I need to replace it?

I downloaded Gsam battery monitor but only shows what apps etc are using power but not battery current health?

theres an app called Battery Info
    https://play.google.com/store/apps/details?id=com.stu.battery&hl=en

it wont give you a percentage of what your battery health is but it will tell you if the health is good or bad. which is enough information to know whether you need a new battery or not.

on some android phones you can check by entering *#*#4636#*#* on the dialer to check the battery health but cyanogen has it removed.

You can try the app CPU-Z. I think it has it. Don't quite remember it.
Anyways the time when ur battery automatically goes from 50 to 0 and phone switches off. That's the time to replace.

Better to get it changed at a service center. Though there are tutorials but officially it's not user replaceable.

You need one special tool to open the case that costs about $2. The battery is less than $10 and changing it takes 15 minutes

My Battery does not last for more than 12 hours anymore. The Display is at 50% an makes most of the usage (10%) with Adnroid OS in second (4%) and Android-System in third (3%) place.
It's "optimized" and the Akku-Info says Health is good.
    Any Ideas?

How about this, just get a new phone! Amazing right?
After a phone gets more than 3 years old, and it's not a removable battery, it really is time to move on, get a new phone.

There's a reason that most new phones do not come with a removable battery. Manufacturers would go out of business if all the customers kept their phones for 4+ years!

Use a wallpaper that has more dark colour like black and less bright colours.
Turnoff navigation when not being used
Don't charge the phone upto 100%
    Don't leave the charging overnight
    Don't play too many games
Rapid discharge then recharge degrades the battery
