I had this H, H+ problem on my Xperia Z with 3 here in the UK. I could only get data if the display showed H+. I have changed to Virgin Mobile for a month to test it and it does not have this problem. I still get data with Virgin even if the display does not show H+. I always suspected it was a problem with 3.

I have the same issue. My phone is on 33r. I see that it switches between the networks i.e. 3g, h, h+ more than other phones AND the transition isnt smooth.

Ive come from an iphone 5 which can manage the change between the networks a lot better and smoother. On the iphone i would only get a similar issue if the signal degraded which is understandable.
The One does switch up and down more frequently while in use, meaning more times i have to refresh a page if it changes signal while loading a page. This is in the same location (my house) as the iphone.

Just got my OPO (chinese) on Three UK and am seeing similar 'issues'. The reason I searched though is because my H and H+ signal doesn't seem to get up over 0.25meg using speedtest. Is this normal? Anything I can do?
