From the start, i noted there's many un-happy incident happens with DHL shipping OPO to Malaysia. Thus, without second thought, i emailed Comgateway (CGW) request to change default courier DHL to FedEx way in advance after i placed my order on OPO.
From observation, FedEx will prepare all the paperwork once pickup the parcel from CGW, which some time might cause some delay in delivery. But this is good, as everything is sorted out when my OPO landing in Malaysia.

Of all the documents, attached is the most important document that you need when submitting Sirim Application.
This document is available once Comgateway declare your parcel to FedEx and when FedEx USA pickup the parcel, they will send the info to Custom Malaysia to determine on taxes.
As you can see, on 24/7/14, cellphone does not attract any taxes, but (for my case) healthy supplement does. RM10.73 is the taxes i need to pay and this prompted FedEx Msia to call me to confirm my agreement to pays RM10.73 taxes.
But the FedEx Msia CS did not ask for Sirim permit, so i asked her whether my other parcel need Sirim permit or not. She re-check again and said YES! Subsequent after the call, she emailed me the PDF documents. The next day 25/7/14, i went to Sirim office to apply for the permit. 
So, either get stuck or be pro-active settle SIRIM in ADVANCE.
