Your bootloader's clearly unlocked and you've somehow seemed to have triggered DM-Verity

Assuming you have experience in rooting and flashing custom ROMs, I'd recommend using the MSM Download tool to flash your device back to stock, a.k.a. the Unbrick tool. If not, simply contact OnePlus via live chat and they'll remotely host a session with you and flash your device for you. Don't forget to make a full backup of everything as your device will be completely wiped during the process.

The same way you unlocked it, using ADB/Fastboot via terminal prompt.

    code:
    fastboot oem lock

Though, if you've modified your devices /System partition in any way I suggest you flash stock OOS 5.0.X and perform a factory reset before doing so.

You could also root the device by flash magisk v19, or flash a custom kernel, to bypass DM-Verity.

TWRP custom recovery is required to perform these actions.

Yeah I wouldn't recommend that tbh. Much safer to stick to the MSM Download tool if you wanna lock the bootloader

My guess is OP unlocked by developer options. I would go for what athaarv suggest or just leave it as is. Also there is nothing you can flash that makes that message go away.

According to my knowledge, your bootloader doesn't unlock simply by checking "OEM unlocking". That only allows it to be unlocked. It's more of a precaution put in place to prevent accidental unlocks unless you definitely know what you're doing. To actually unlock it you need to run an ADB command after enabling OEM unlocking

    Edit: Fixed formatting error

You are correct. Checking the box simply allows for the unlock. The command has to be run for it to actually unlock.

To be honest, ive gotten this dm-verity screen on a totally stock OOS, bootloader locked. Bootloader unlocking shows a different screen with yellow text, right?

And running a command to reenable dm verity works for a while, then the whole scenario occurs again. Any ideas?

I used to get this warning whenever I was reverting back to Oxygen OS. Using the adb command worked for me.
