Yes slidder is in the correct position. I have reduced the amount of apps on phone to a minimum and it's still doing it driving me crazy as only indication of receiving a call is screen lights up

It goes to actual silent mode or does it still vibrates when you get notifications as it would if you set the slider in the vibrate position?

I've had the same thing happening to me, turns out i accidentally pressed the power and volume up button at the same time, which puts the phone in silent mode, but it would still vibrate. I also had to change settings on my smartwatch when i bought one, because it happened then too, something with bluetooth connectivity

My seems to have a mind of its own and switches onto silent at anytime even at night when phone is just sitting there so I can't blame myself and to go into both silent and not to vibrate is just very annoying. Pretty poor response from OnePlus I must say

I've found this on Google, it seemed to fix it for a lot of people, don't know if it'll work or if you or the customer support already tried it.

Maybe a stupid question but could it be Do Not Disturb is switched on or a planning for DND set?

No not a do not disturb, I have also completely wiped clean the phone on numerous occasions and when it gets sent for repair it has to be completely wiped clean so on reinstalling it goes back to factory settings.
