I've attached two screenshots for examples where you can see the Reading mode toggled on and off. Obviously the Reading mode is not monochrome anymore. What's going on?

You have to toggle it from the quick settings. Or add an app to automatically turn on reading mode for.

Thanks for the reply. However I see Night mode, but not Reading Mode in the Quick settings. I'm asking about Reading mode and why it is not monochrome anymore.

Get to your quick settings, tap the pencil icon, then drag the reading mode icon into your quick settings panel.

Thanks! So I was able to modify my Quick Settings. That's a big help in itself :)
I used it to turn on Reading mode. The thing is Reading mode is not grayscale on my phone. It gives the screen a reddish tinge (removes the blue light, I'm guessing).
The screen does a weird thing when it's turned on - it slowly changes to grayscale and then color fades back in, and definitely less harsh on the eye.
Is this supposed to be the way it works?

Do you mean that the color fading back in is that it's slightly yellowish?

No. All the colors are clearly discernable. Its similar to what apps like Twilight do.
So definitely not grayscale like it used to be. I bought this phone in January and Reading mode used to be a yellowish/reddish monochromatic colour scheme. Similar to an old newspaper. That's not the case when I tried it today.

Go into your reading mode settings, and add whatever app you want to be grey scale.

reboot to recovery and wipe cache:

    1. Power off
    2. ‎Power + Volume down button - hold this combination till device starts
    3. ‎English (unless you know one of the other languages)
    4. ‎Wipe data and cache
    5. ‎Wipe cache (don't choose the other options)
    6. ‎reboot

Just tried that. None of the apps I'm adding are in grayscale.
What happens is that they open up in grayscale and then the color fades in.. so they're fully colored but less harshly so.

That's quite odd. Try clearing your cache as suggested by @Sridhar Ananthanarayanan.

I powered off. Restarted holding down the Volume and Power buttons. But I didn't get taken to any recovery screen or have to choose any options.
The phone just started up (rather quickly, I might add) and since I had powered off with Reading mode on, it was still on when it restarted.

And it's B/W now! Thanks guys. It's fixed!

One more question, if I may: any idea if this screen mode improves or harms battery usage?

It's the same for me on my 3T. Sometimes it's normal grayscale & sometimes it gets a touch of yellowish, but yes I can easily distinguish the colors...

Thought that it was a 3T exclusive bug, but it's an OxygenOS bug actually :/

I don't think it positively or negatively affects battery life.
    Good to hear!
