Tbh, this all looks kind of weird indeed, my best opinion is the battery has come to its end. Best would be to change battery if you have such strange readings.
If possible, I would go to a service centre and let them test the battery in a proper way. Another thing you could do first is to run your device in safe mode for a while to see what is happening.

Also, for the time backup your important files if not done so as there might be a possibility the battery stops working suddenly.

Before you change the battery have you tried clearing the cache and if that doesn't work try it in safe mode. Usually batteries fail gradually rather than instantly have you notice a slow deterioration?

Could you provide your new phone screenshot for comparison?
Battery chemistry is very specific, and batteries from a same batch won't show same figures.
And measuring tools may have some variations too.

A modern battery is built for about 500 cycles and 4 years lifespan (when it might still have 50-60% of its initial capacity).
If you battery lost 10% in more than 2 years, maybe 2,5 years (you don't know its manufacturing date, only your purchase date), it's not surprising.

Is that feature built in, or is it a app your using for that? If so, what's the app name?

My oneplus 3T has 86% battery health after about 2 years and 2 months of use. I'm able to get about 3.5 hours of SoT and about 4 hours if i trihard. I've done all the suggested methods mentioned in this thread. My question is - is there is a way of improving my current battery life and get some more SoT out of my battery? Or is this kind of SoT normal?
My usage includes 20 mins of social NW in a day, light gaming of about 30 mins (no, not PUBG), listening to music for like about an hour or two and a lot of browsing on chrome.
I've used Accubattery to check my battery health.
    Any suggestions are welcome.

Watching YouTube doesn't consume much battery. The same goes for streaming offline videos too. Even I get an SoT of about 6 hours if I stream videos off YouTube. Streaming videos isn't the usage scenario that everybody has. There are voice calls, social NWing, a bit of gaming, browsing etc.
