    Doesn't look much...but,
    Umm....yes ?

Depending on how much of a risk you're willing to take....

Would you sacrifice your phone for a charger ?

The reason is in your wall socket. Contacts inside of the wall socket are getting loose and due the bad contact causing sparks. I would call immediately to certified electrician to change the wall socket before your house get burnt.
If you are using extension cord throw it away and get the new one.

I have seen multiple devices getting damaged because of loose contacts in the sockets.

I have to correct my previous post little bit. My opinion is still the reason is in your wall socket or in your extension cord but I don't want to exclude your charger. It may also being damaged, but I would be very careful with the wall sockets since loose connection may cause a fire
